Our leaders of the Camp will exercise all reasonable supervision and care of the children attending the Camp. However, in the event of your child sustaining injury or becoming ill during the Camp, it is necessary to have your authority for medical aid to be enlisted in such circumstances and for you to indemnify the leaders including the North Ryde Christian Church and the Winmalee Christian Conference Centre against liability and the cost of medical fees. Accordingly we would ask you to kindly complete the undermentioned authority and indemnity. I hereby authorise the leaders of the Camp and such other associated persons, that in the event of my child sustaining injury or becoming ill, to enlist medical assistance and I hereby indemnify and hold harmless such persons including the North Ryde Christian Church and the Winmalee Christian Conference Centre against liability and the resultant costs incurred.
